Surrounded by memories, familiar furnishings and family photographs, people almost always wish to confront their issues of illness, accidents, aging, and even dying in the comfort of their own homes. Mercy Home Health serves individuals of all ages who are ill or injured and have the need for skilled health care services at home. The most common conditions served include congestive heart failure, emphysema, diabetes, vascular disease, muscle disorders, wounds and recovery from joint replacement.

Mercy Home Health also provides services to clients who do not have a need for skilled care but need assistance with personal care and other activities to allow them to remain in the home. Whether providing skilled health care services or simple bathing assistance, Mercy Home Health’s goals for clients include avoiding or limiting hospitalization, thereby improving their quality of life and helping them achieve independence in their own homes.

Our services

Under the direction of a patient’s physician, Mercy Home Health provides skilled services, equipment and specially trained staff.

  • Registered nurses on call 24 hours a day, provide a wide range of skilled services including cardiopulmonary assessment, diabetes education, medication education, wound care and intravenous therapy. Specially trained nurses provide palliative and transitional care, as well as comfort care services including pain and symptom management.
  • Physical and occupational therapists set up home exercise programs for people following joint replacement surgery, injury or serious illness. Occupational therapists help patients maximize their independence at home by evaluating their ability to care for themselves, making recommendations for equipment, developing home exercise programs, and coordinating home care modifications, supplies and equipment.
  • Mercy Home Health telemonitoring service manages a patient’s health information seven days a week, and encourages patients to practice preventive and proactive self-care. Telemonitoring is cost effective, provides early detection of health problems, and offers a sense of security for patients and families.
  • Speech therapy is available to patients with speech and swallowing difficulties. A certified speech and language pathologist will diagnose the difficulty and develop an individualized care plan tailored to the patient’s needs.
  • Medical social workers assist patients and families with long-term planning such as advance directives, alternative placement and continuum of care plans. They also help by accessing community resources such as funding options, adult day care, Meals on Wheels and many other community programs.
  • Home health aides assist patients while they recover from illness or injury. Home health aides offer important personal care including bathing, dressing, transferring, feeding, assistance with medically-oriented tasks and exercise routines, all while promoting independence.
  • Personal care aides also provide assistance with personal care services, including bathing, grooming, transferring, meal preparation and more for clients eligible under the Wisconsin Medical Assistance program.

Home companion services

In addition to Home Health services, Mercy Assisted Care offers Home Companion services. These services allow the patient to maintain a safe and healthy home environment by receiving help with shopping, errands, cooking, chores, cleaning and other household tasks. Payment for these services includes county funding, grants or private pay.

Referral and evaluation information

Any individual can request home health services. Referrals may come through the hospital discharge planner, a social worker, physician or nurse. Additionally, you or a family member may request an evaluation for home health care eligibility.

Medicare, medical insurance, private insurance, county funding, grants and private pay cover home health services. A nurse or therapist will perform an evaluation visit to determine if you are eligible for services, as well as discuss your payment options.
