Our services
Skilled medical assistance — If you are recovering from major surgery, Mercy Home Health Care staff will visit and help with medications, wound care, physical and occupational therapy, and other skilled services.
Personal care assistance — Our personal care aides are here to help you with important everyday tasks like bathing, washing your hair and getting dressed.
Homemaking assistance — Do you need help with errands, cooking, small chores, cleaning or other household tasks? Consider our Homemaker Companion Service.
Mercy Home Health Care telemonitoring service — Telemonitoring manages your health information seven days a week in the comfort of your home. It also encourages you to practice preventive and proactive self-care. Telemonitoring is cost effective, provides early detection of health problems and offers a sense of security.
Private home care — Private home care is available when you need special care to live safely and comfortably at home regardless of your age or disability. This is a self-pay service.
Our team
Mercy Home Health Care’s specially trained staff cares for you under the direction of your doctor, and includes:
Registered nurses, on call 24 hours a day, provide a wide range of skilled services including cardiopulmonary assessment, diabetes education, medication education, wound care and intravenous therapy. Specially trained nurses provide palliative and transitional care, as well as comfort care services including pain and symptom management.
Physical and occupational therapists set up home exercise programs for people following joint replacement surgery, injury or serious illness. Occupational therapists help you maximize your independence at home by evaluating your ability to care for yourself, making recommendations for equipment, developing home exercise programs, and coordinating home care modifications, supplies and equipment.
Speech therapists help you with speech and swallowing difficulties. A certified speech and language pathologist diagnoses your difficulty and develops an individualized care plan tailored to your needs.
Medical social workers help you and your family with long-term planning such as advance directives, alternative placement and continuum of care plans. They also can connect you to important community resources such as funding options, adult day care, Meals On Wheels and other community programs.
Home health aides help you while you recover from illness or injury. Home health aides offer important personal care including bathing, dressing, transferring, feeding, assistance with medically oriented tasks and exercise routines, all while promoting your independence.
Personal care aides also provide assistance with your personal care services, including bathing, grooming, transferring, meal preparation and more if you are eligible under the Wisconsin Medical Assistance program.
Journey Program
Sometimes, despite everyone’s best efforts, your chronic illness will progress and cause severe symptoms like pain, breathing difficulties, fatigue and nausea. When this happen, Mercy Home Health Care’s Journey Program is committed to maintaining your quality of life as long as possible.
Our Journey Program staff helps comfort you by managing your various symptoms. They even offer you at-home chemotherapy and pain management treatments.
Our nurses are cross-trained in hospice care, so if your condition worsens, they can gently guide you toward any hospice care setting, including your home.
Referral and evaluation information
Any individual can request home health services. Referrals may come through the hospital discharge planner, a social worker, physician or nurse. Additionally, you or a family member can ask for an evaluation for home health care eligibility.
Medicare, medical insurance, private insurance, county funding, grants and private pay cover home health services. A nurse or therapist will perform an evaluation visit to determine if you are eligible for services, as well as discuss your payment options.
Mercy Home Health Care is state licensed and Medicare/Medicaid certified.
